To become a new patient, please do the following:
 
1. Please either have your Primary Care Provider or Therapist fax us a referral at (971) 239-4271. Alternatively, you may text us directly at (828) 544-5101 to initiate the process. Please provide us with your name, Date of Birth and email address. 

2. You will be emailed an invite to create a patient portal and complete our intake packet. 

3. Once completed, our referral coordinator will contact you directly to schedule an appointment. 

***Please have your PCP and/ or most recent psychiatric prescriber fax recent visit notes to 971-239-4271

***We are ONLY accepting clients for medication management or Transcranial Magnetic Stimulation (TMS) at this time

***We are typically able to schedule intakes within 3 weeks, once all paperwork is completed
